Patient Centered Medical Home is a model of care that emphasizes care coordination, enhanced access and patient centered interactions, which provides patients with the healthcare they desire. MAeHC has developed proven approaches and tools to assist practices in becoming successful Patient Centered Medical Homes or Patient Centered Specialty Practices. We have assisted over two hundred providers with redesigning their clinical and administrative processes to improve clinical quality, patient experience, and provider and staff satisfaction. MAeHC's collaborative approach provides education on the concepts of a medical home, and provides the nuts-and-bolts tools to help a practice transform.
Through an initial assessment tool that aligns with the NCQA PCMH Standards and Guidelines, we will identify gaps and provide future-state recommendations. Our consultants will work with you to develop a detailed analysis, work plan and timeline that will provide you with a roadmap to support your transformation to becoming a Patient Centered Medical Home. MAeHC uses a combination of onsite meetings and virtual assistance using conference calls and web-based sessions to meet the needs of your practice. The MAeHC team currently has NCQA Certified Content Experts on staff to assist organizations with the practice transformation and navigating the requirements to achieve PCMH Recognition under the NCQA Standards.
